Export transaction list including both the category and account for each transaction

I would like to export a list of transactions that incudes for each transaction both the category that has been assigned to the transaction and the account in which the transaction occurred. The column labels would be:
. . . . . . . . . . . .
Date | Description | Account | Category | Amount | Notes
. . . . . . . . . . . .
I can generate reports and export files that list transactions by account or by category, but I cannot find a way to generate a report or export a file that includes both the account and the category for each transaction. Is this possible?
